USAvision & Your Privacy

USAvision values and respects the privacy of the various publics who visit its Web site, usavision.net, including guests, health plan members, health care providers, group benefit administrators and authorized producers.

This privacy statement discloses the information collection and handling practices of USAvision in relation to usavision.net and all other Web sites owned and operated by USAvision and its subsidiaries. 3. Collection of personal information
USAvision may collect and/or track the following information from Web site visitors:

3.1 Domain names, type and version of Web browser used, type of computer and operating system used, pages accessed, files downloaded, and search engine used by site visitors;

3.2 The email addresses of Web site visitors who communicate with USAvision via email;
Information knowingly provided by the site visitor via online forms or surveys (including demographic and personal data);

3.3 Aggregate and user-specific information on pages accessed by Web site visitors (see heading "use of cookies and related technology";

3.4 Information provided by visitors utilizing USAvision's Web site search tools. When a visitor performs a search within USAvision's Web site, USAvision may record information identifying the visitor or linking the visitor to the search performed. USAvision may also record limited information from each search request and use that information only to solve technical problems and calculate overall Web site usage statistics.


4. Use of "cookies" and related technology
USAvision may place "cookies" on visitors hard disk drives to allow USAvision to customize and enhance each visitors experience at usavision.net, make improvements to the site and report site activity. These cookies never will be used by USAvision to track visitors activity on any third party Web sites or to send visitors unsolicited e-mail messages, or "spam" When a visitor returns to USAvision's Web site, USAvision may recognize the visitor by the cookie and customize the visitor's experience accordingly. Visitors who choose to do so may configure their Web browser software to decline the use of cookies. However, application functionality may be affected if the use of cookies is declined.

5. Use of personal data collected
Personal data collected by
USAvision may be used for multiple reasons including editorial and feedback purposes, marketing and promotional purposes, statistical analyses of visitors behavior, product development, content improvement, or the customization of the content and layout of USAvisio's Web site. Aggregate data on visitors domain servers may be used for internal purposes such as maintaining Web site usage statistics. Individually identifying information, such as names, postal and email addresses, phone numbers, and other personal information which visitors voluntarily provide to USAvision may be added to USAvision's databases and used for future calls and mailings regarding Web site updates, new products and services, and upcoming events.

For example, all USAvision employees are required to certify their understanding that personal information is considered confidential, that it is important to safeguard personal information, and that USAvision will take appropriate action against any employee who fails to acknowledge these facts or adhere to the code of business conduct. Please be advised, however, that the confidentiality of any communication or material transmitted to or from USAvision via the Internet (World Wide Web, email or other) cannot be guaranteed. Accordingly, USAvision is not responsible for the security of information transmitted via the Internet. 8. Collection of personal data from children
To the extent that
USAvision is able to determine the age of users by their submissions to or communications with USAvision's Web site, USAvision will not knowingly:

8.1 Collect or post information from individuals under the age of sixteen ("children") without their parent's or guardian's consent, or

8.2 Provide any personally identifying information collected from children, regardless of its source, to any third party for any purpose whatsoever. If registration is required for any services on the USAvision site, and a user submits information to USAvision's Web site through such a registration process that indicates that such user is a child, the child's parent or guardian will be alerted to the child's registration and will be provided instructions on how the parent or guardian can delete the child's registration, if desired. No information collected through USAvisio's Web site from users identifying themselves as children will knowingly be used for any direct marketing or promotional purposes.

9. USAvision's right to contact visitors
USAvision reserves the right to contact site visitors regarding account status and changes to the subscriber agreement, privacy statement, or any other policies or agreements relevant to site visitors.

10. USAvision's right to change privacy statement
